Step 1: Captains please sign-up your team!

Step 2: Each team member must print waiver and bring it to the event!

Step 3: Each team member, please make a payment.

Individual Payment to Participate:

  • $30.00 per person, but if you sign by May 11,2010, it is only $25.00 per person 
  • A spot is held for a team only if ALL members on a team have paid.

**All Players must check-in/register at 10:00am on May 22nd.  If a team or player failes to check-in/register before the start of the tournament they forfeit the right to play and money donated will not be refunded.
